Effective Fall 2007 Semester, students who enroll in college for the first time during the fall 2007 academic term or any term subsequent to the fall 2007 term, an institution of higher education may not permit an undergraduate student to drop a total of more than six (6) courses, including any course a transfer student has dropped at another institution of higher education.

Drop Notification Report - Use to drop a student for excessive absences. For other than excessive absences, use the Administrative Drop Report.
Administrative Drop Report - Use for other than excessive absences (plagiarism, disruptive behavior, etc.)
Reinstatement Report - Use to reinstate a student. Should be submitted within 7 days of the Drop Notification.
Student Schedule Change - Use before the Official Census Date ONLY.
Student Initiated Drop Form - Use after the Official Census Date ONLY.
Grade Change Form - Can not be accessed online. It has to be requested via email or by calling ext. 2574.
Incomplete Grade Form - Has to be signed by student, instructor, department chair, and instructional dean.
Degree Requirement Change - Has to be approved by department chair and department dean.
Class Schedule Change - Has to be approved by department chair and department dean.
Credit By Institutional Examination - Has to be approved by department chair and department dean.
Hold Notice - Use to place a Hold on the student's record.
